Seminar Camp Application Directions

Application for Seminar Camp (print 1 copy)
Youth Delegate Reference Form (print 2 copies)

• Deadline for applications is December 10, 2009. We appreciate having applications in as early as possible to help us plan for selection. If you are past the deadline, please contact Kerry McClure to see if we are still accepting applications.


• Enclose $25 check made payable to CISV for the application


• Once completed, mail check and Seminar Camp Application to:

References

Provide two references:

• One from the applicant's current teacher


• One from an adult who knows the applicant in a non-school setting (coach, youth leader, employer, family friend, etc.)

On Youth Delegate Reference Forms, fill in:

• Youth's name


• Write in "Seminar Camp" under "youth is applying for" (not listed as an option on this form)

Due date of the reference forms is December 10, 2009.

Provide the referees (persons providing your references) with an envelope with the address above printed on it, so that they may send it directly to the CISV Seminar Camp Selection Chair. Referees must complete and mail reference forms to the Seminar Camp Selection Chair.
