Summer Camp Application Directions

Youth Delegate Application (print 1 copy)
Supplement for Summer Camp Applicants (print 1 copy)
Youth Delegate Reference Form (print 2 copies)

• Deadline for applications is December 10, 2009. We appreciate having applications in as early as possible to help us plan for selection. If you are past the deadline, please contact Adam Robinson(adam@cpasite.com), or Eleanor Fails-Hayden (failse@aol.com), Recruiting Committee, to see if applications are still being accepted.

• Make sure to include a photo on Supplement for Summer Camp Applicant form

• Enclose $25 application fee made out to CISV

• Once Completed, mail: check, Youth Delegate Application and Supplement for Summer Camp Applicant to:
  CISV Summer Camp Selection Committee
  CISV Jacksonville
  2148 Mango Place
  Jacksonville, FL 32207

References

Provide two references:

• One from the child's current teacher

• One from an adult who knows your child in a non-school setting (coach, youth leader, family friend, etc.)

On Youth Delegate Reference Forms, fill in:
Child's name
Check Summer Camp box under "youth is applying for"
Due date of form is December 10, 2009.
In box for "Please return form to", write:

CISV Summer Camp Selection Committee
CISV Jacksonville

Jacksonville, FL 32207

Provide the referees (persons providing your reference) with an envelope with the above address printed on it, so that they may send it directly to the CISV Selection Committee. Referees must complete and mail reference forms to the CISV Selection Committee.

Scholarship Application

The deadline to apply for need-based financial aid is December,10, 2009. The forms can be obtained by contacting Patty Pritchard (jaxcisv@bellsouth). Please direct questions to Patty at (904) 306-0820.
